Remote Monitoring@Home (RM@H) develops hybrid care pathways involving hospitals, general practitioners, care organisations and patients. By combining remote technology via wearables and improve communication between primary and secondary care it will ultimately make patient care better, more personal, and more efficient.
Healthcare systems in the Meuse-Rhine region are known for their high quality. However, care that continues from the hospital into the community, called transmural care, still has its challenges. Patients with chronic illnesses are often only transmurally monitored occasionally, leading to frequent hospital readmissions. Such a situation only puts extra burden on patients and adds more pressure on hospitals but it also leaves significant gaps in care provided by general practitioners and care organisations. This happens because they cannot communicate both ways or get information about the patient’s health at home.
RM@H’s ambition is to bridge these gaps by continuously monitoring patients at home and improving data flow between primary care (general practitioners and care organisations) and secondary care (hospitals). The project focuses on studying the current care paths for specific patient groups, including those with breathing problems, patients recovering from surgery, cancer patients, and psychiatric patients. Using this information, RM@H seeks to develop a novel, hybrid care path (HCP) per patient group. These paths will involve insights and expertise from all stakeholders, including primary and secondary care providers and most importantly patients.
By using remote monitoring and involving primary care more, RM@H aims to shift healthcare from reactive "sick care" to proactive "health care." This will lead to better health outcomes for patients and make healthcare systems in the Meuse-Rhine region more effective. Supported by 12 dedicated partners, RM@H is working to create an integrated, efficient, and patient-centred healthcare system.
